RN & LVN Field Visits

We are looking for a talented Psychiatric Registered Nurse Case Manager to join our team at Abacus Home Healthcare. Ideal candidate would have experience with Kinnser (Wellsky) Software, be familiar with OASIS documentation, and be autonomous in their psychiatric nursing duties within the home environment. Most patients' psychiatric diagnosis will be related to Alzheimer's, Dementia, Bipolar Disorder, Significant Depression, and other primary psychiatric diagnoses. RN should be familiar with home health coding and workflow requirements to properly manage a patients diagnoses' at home. This includes case managing patients with psychiatric diagnosis as well as other diagnoses, including woundcare, ostomy care, CHF, COPD, UTI, etc. The Case Manager must be willing to see ALL types of patients. Our company believes in the true case management model in which our nurses coordinate their own care with the interdisciplinary team members, including therapy, social work, and home health aide. Our service area consists of Rockwall to Mt. Pleasant, Commerce, Mineola, Holly Lake Ranch and north and south of those areas all the way to Hwy 80.

Productivity Expectations:

Our Full-Time RN Case Managers are required to complete a quota of 25 points per workweek (M-F 8-5pm). Company will begin paying PRN rates for points completed over 30 per workweek. RN will be required to rotate ON-CALL for one week at a time for $150 per week. ON-CALL starts on Fridays at 5pm for the weekend and at 5pm on each weekday. RN will be required to triage calls and perform after hours visits if necessary. Currently, RN should be in rotation every 6 weeks. PRN points are Admit OASIS: 3 points or $90, DC/RECERT/ROC: 2 points or $60, Routine Visit: 1 point or $45


Company currently does not cover group health plan, but does try to be competitive in pay in order for employee to afford individual health insurance. This could change within the next year.


Company provides 3 days PTO in 1st year, 5 days in 2nd year, and 10 days in 3rd year of service

Phone and Car:

Company provide a phone allowance of $50 per month and car or a car allowance, depending on the situation of the employee with their own personal transportation.
