General Responsibilities

Performs a variety of duties to provide quality nursing care to residents. Coordinates total nursing care for residents. Collaborates and works with the DON and Administrator to establish goals, develops and implements procedures and ensures the smooth, efficient and safe delivery of care and services to residents. Ensures compliance with all operating policies and procedures, the Department of Health, the Pennsylvania Department of Public Welfare and other regulatory requirements. Ensures ongoing communication with supervisors, provides daily reports and other periodic reports. Performs all the duties of CNA s when required, supervises licensed staff, CNA s and nursing ancillary staff. Provides exceptional customer service.

Essential Duties

Conducts resident interviews upon admission and develops appropriate care plans.

Monitors residents conditions and obtains information to develop appropriate interventions to prevent decline and to support the highest level of functioning and communicates appropriately.

Responds to emergencies in an expedient manner. Calls appropriate physician and ambulance service if needed. Confers with residents and families respectfully answering questions and giving timely and accurate updates of information related to the care, treatment and condition of the resident. Notifies the resident s family when resident has a change of condition, fall or when resident is transferred or admitted to the hospital.

Maintains the medication administration system, including account for medications, verify monthly medication physician orders. Administers medication and treatments to residents within medication administration protocols as needed. Places orders for medication from the pharmacy.

Completes resident incident report forms in a timely and accurate manner. Conducts appropriate follow up investigation of issues and provides documentation of such. Promptly shares verbal and written report of incidents with supervisor.

Makes daily rounds on all residents and assures that all care and service needs are met.

Ensures that residents are aware of the services that are available to them. Encourages residents to participate in activities program and provides staff support to enable residents to get to and from and participate in activities.

Maintains accountability for controlled substances. Ensures that all protocols for administration of controlled substances are strictly adhered to . Inventories medications at the beginning and end of the shift and provides accurate documentation of medication inventory. Notifies Director of Nursing upon discovery of any potential discrepancies.

Communicates the status of residents to incoming personnel according to established procedures; communicates with physicians; transcribes, clarifies, and implements physicians orders. Reads all communication logs and follows through as indicated. Effectively communicates with external providers including; pharmacy, physicians, rehab, special needs, etc.

Assists with orienting new team members. Supervises Certified Nurse Assistants and determines work procedures, prepares work schedules and expedites work flow.

Ensures all care providers document all care provided to residents in CareTracker in an accurate and timely manner.

Adheres to all universal precautions and prescribed safety and infection control procedures at all times.

Basic Qualifications

Experience: A minimum of one year s experience in related licensed nursing positions. Prior supervisory or charge experience preferred.

Education/Training: Graduate of an approved school of nursing; current Pennsylvania license to practice as a licensed practical nurse or a registered nurse.
