Job Description


We seek an RN Case Manager to serve our Veterans in Cecil County County. The RN is a professional caregiver who assumes responsibility and accountability for a group of assigned patients in a home setting, rendering professional care in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, implementation, and evaluation. He or She will , provide case management, addressing physical, psychosocial, and spiritual aspects of care. The RN is a clinical practitioner who coordinates and implements patient care specific to the population served.

Job Requirements
  • Registered Nurse (RN)
  • License: Registered Nurse (RN) License in Maryland (Required) Requirements Excellent oral, written and interpersonal communication skills. Demonstrated emotional maturity and ability to cope and work effectively in stressful situations. Demonstrated ability to work independently with minimal supervision. Experience: nursing (home care): 2 years (Preferred) nursing (hospice): 1 year (Preferred) nursing (acute care): 2 years (Required)
