Job Description

On Lok Lifeways PACE long term senior care is provided by On Lok Senior Health Services, a 501(c)(3) non-profit On Lok, Inc. is a not-for-profit organization founded in the early 1970s by a group of citizens concerned about the plight of elders and the lack of long term options in the community. On Lok, Inc. serves as the administrative arm of a group of five affiliated non-profit organizations. POSITION SUMMARY: Under the administrative supervision of the Program/Center Manager, the clinical direction of a Center RN, and the clinical supervision of the associate nurse manager, the Licensed Vocational Nurse provides direct nursing services to On Lok participants.

  • 1. Processes, communicates and implements all medication orders.
  • 2. Administers and distributes all medications as prescribed by On Lok medical staff.
  • 3. Provides nursing care and treatments for participants at the Day Health Center.
  • 4. Maintain clinic supplies
  • 5. Participates actively in the interdisciplinary team by offering clinical expertise in team planning, problem solving, and processes.
  • 6. Teaches, supervises, and counsels the participant, geriatric aides, and family members regarding nursing care needs and other related problems of the participant.
  • 7. Other related duties as assigned.
  • QUALIFICATIONS (knowledge, skills, abilities):
  • 1. Graduate of an accredited LVN program.
  • 2. Possess a current California LVN license.
  • 3. Interest, demonstrated ability and one-year experience working with the elderly or frail population required.
  • 4. Ability to work independently and as part of multidisciplinary team.
  • 5. Bilingual ability may be required for specific assignments.
