Job Description

CareHere is currently seeking a full time LPN or RN at our onsite medical clinic in the Greeley, CO area for 30hours per week - Monday 7:30am - 3:30pm, Tuesday 8:00am - 12:00pm, Wednesday 7:30am - 3:30pm, Thursday 8:00am - 2:00pm, Friday 7:30am - 12:30pm .

  • We are looking for a passionate Nurse who believes in putting their patients first and values the patient- relationship to help engage them in their health risks, goals, objectives and work collaboratively with the provider to engage the patient in maintaining or improving their health.

    Primary Responsibilities:

  • Greet patients upon entry to health center and respond to patient calls.

  • Schedule appointments for patients as needed and assist patients with electronic access.

  • Provide intake including blood pressure, temperature, pulse, respirations, waist circumference measurement, height, weight, chief complaint, medication reconciliation and health history.

  • Prepare patients for exam with provider.

  • Document data collection in the Electronic Health Record.

  • Assist the provider and/or other nursing staff with procedures.

  • Administer medications as ordered by provider.

  • Keep environment clean and disinfected after each patient use.

  • Collection and handling of laboratory specimens.

  • Assist in maintaining medical inventory and supplies.

  • Perform EKG's and other in office patient testing as identified.

  • Reinforce patient education as directed by provider.

  • Coordination and tracking of referrals.

  • Assist with audits and regulatory functions.

  • Required Qualifications:

  • Maintains current licensure for LPN/LVN or RN in state where practicing

  • Computer literacy, including with MS Office, electronic health records, internet, and mobile applications

  • A friendly personality with good people skills including team collaboration

  • 2 years' recent experience in Nursing

  • Proficient phlebotomy skills

  • Current BLS CPR certification
